I love this exercise because the gluteus medius muscle is so important for the rehabilitation of a patient with sciatica and lower back pain. Most commonly, as I mentioned above, people rotate their hips and the exercise will be ineffective. That is why it is of the utmost importance to make sure that you do not make that mistake.

Sciatic Nerve Mobilization

Now that the surrounding tissues are stretched, it’s important to perform some dynamic exercises that allow the sciatic nerve to be mobilized. Jeong et al. determined that, “Application of mobilization techniques for the sciatic nerves may promote healing of the soft tissues by stimulating the functions of the nervous system to improve nervous system adaptability and decrease sensitivity, helping to alleviate the symptoms.” The group that experienced the most relief in this study performed these mobilization techniques in addition to other therapy exercises for lumbar stabilization. Therefore, I like to include these in my treatment plan along with some other treatments I will go over shortly.

It is important for you to realize that you can absolutely overstretch and cause additional pain with these exercises. That is why I always tell people to perform them right before they begin to feel pain and discomfort. The tissues being stretched and mobilized during these exercises are very sensitive, and flaring them up more is not the goal here. You also do not want to do more than 10 repetitions per day due to the sensitive nature of these exercises.
